AEGG LLC's/Chef Alison's classes/camps are intended for your education/enjoyment. All food must be consumed in class and will not be sent home with students. Although classes are conducted under the direction and supervision of AEGG LLC/Alison Garen, food is prepared by the students participating in the class/camp. Your/your child's participation is strictly voluntary.
Because culinary activities can present risks, AEGG LLC/Chef Alison depends on you to use your good judgement and reasonable care for your own safety and the safety of others. Please follow the directions of the chef carefully. If you have any questions, whatsoever, about your ability to safely follow any instructions you are given, please immediately alert Chef Alison.
By signing this form you acknowledge, agree, and understand that your participation is voluntary and you assume the risks associated with the culinary activity in which you will be participating. Such risks may include, but are not limited to, physical injury, foodborne illness, contraction of illness from other participants, allergic reaction or other harm. The kitchen is not allergen free.
I acknowledge and assume all risks associated with the cooking camp, including, without limitation, cuts, burns, food poisoning, slipping and falling, any other personal injury and equipment and facility conditions. I have read and fully understand this waiver and in consideration of the acceptance of entry, for myself and anyone legally acting on my behalf and waive and release AEGG LLC/Chef Alison Garen from any and all claims, liabilities or causes of action, including without limitation, death, bodily injury, property damage, or any other loss, damage or inconvenience whatsoever, arising from my participation in the cooking class/camp.
Further, I grant full permission to use my name, photograph, likeness, biography, voice and/or video for advertising and promotional purposes, including, but not limited to, online and on-air announcements, signage, posters, magazine articles, websites, social media sites and any other publication(s) at AEGG LLC's sole discretion, worldwide and without limitation and without additional compensation or consideration, except where prohibited by law.
